Malaysia's nephrology services have expanded rapidly, yet access, quality and outcomes remain uneven across states and settings. This lecture reviews the current landscape and proposes a service model built on regional networks, task-sharing and stronger multidisciplinary teams.

Service transformation should also prioritise upstream CKD prevention, systematic albuminuria testing, risk-based referral, greater use of peritoneal and home dialysis, strengthened transplantation networks, genomic diagnostics and post-AKI follow-up. Digital platforms, tele-nephrology, registries and quality dashboards can extend specialist reach and enable continuous benchmarking.

The future of Malaysian nephrology depends not simply on increasing workforce numbers, but on ensuring that every patient receives the right expertise, at the right place and at the right time.
